Dick Gin, A.T.C., a native of San Luis Obispo, graduated from California State University, Fresno and M.E.D. from the University of San Francisco. In addition to his work in the clinic, Dick provides athletic training services to high schools around the county. He has served as head athletic trainer for the San Francisco Giants, Class A baseball team and often lectures on the care and prevention of athletic injuries. Dick is a certified member of the National Athletic Trainers Association and the California Athletic Trainers Association.

 
    Don Olson, P.T. graduated in 1978 from the Physical Therapy Program at California State University, Long Beach. His experience as a four-time All-Conference baseball player who survived rotator cuff injury motivated his interest in sports medicine rehab. Don has expertise in extremity rehabilitation, with special interests in the throwing shoulder and golf related injuries. (Don is a single digit handicap golfer himself.) He is a member of the Private Practice, Sports Orthopedic and Aquatic Sections of the American Physical Therapy Association.

T.C. Chaffey, B. S., M. S., is a Cal Poly graduate living in San Luis Obispo county since 1978. T.C. has been with Creekside Physical therapy for the past five years. She heads up the aquatic exercise program utilizing her extensive teaching, coaching and work experience in the aquatic environment. T.C. was a competitive masters swimmer, triathlete and marathon runner until a traumatic fall resulted in a multi-level cervical fusion surgery.     Karen Empey P.T., is a Cal Poly alumnus in Biological Science and graduated from Daemen College, Buffalo, New York in 1994 with a degree in Physical Therapy and a minor in Biology. She has practiced in San Luis Obispo County since 1994 in a variety of settings including acute care to outpatient orthopedic practice. She enjoys post-operative rehabilitation as well as geriatric patient care. She has played competitive sports including soccer and volleyball and has a passion for fitness as well as health and nutrition. Karen is a member of the American Physical Therapy Association.

Shelly Chisamore has been with our clinic for 9 years. She is responsible for managing our front office proceedings which includes scheduling, billing questions, and insurance authorizations. Shelly brings along with happy, friendly face, a diverse insurance knowledge and is available to help you with any questions that may arise during your treatment.   shelly

 

 

 

Mo   Maureen Jensen, a native of Morro Bay, is our newest member to the team.  She brings a vast variety of medical office experience to our clinic.  Her responsibilities include greeting and scheduling patients, as well as insurance verification and authorization.  Her outgoing, friendly, and caring personality is a valuable addition to our front office management. Each patient is greeted with compassion, warmth and always a smile.  “Moe” as she likes to be called, is a welcome addition to our family.
